  1. Product code and description of product
  2. Production code date and expiration date (located on the side of the bottle)
  3. Number of cases involved
  4. Name of distributor and Representative (if applicable)
  5. Description of problem
  6. Name, address, and phone number of contact

Please provide any other information that would help us better investigate the problem (i.e. how product is being handled, cooked, photos, etc.)
